How does one tighten up the now sagging skin after weight

I have taken off 45 pounds and the skin is sagging and hanging on my belly, thighs, arms, and neck especially. It gets better on it's own, if the excess weight has been there for a very long time, like 10-20 years, the skin is very distressed and cannot be expected to snap back 100%. And, luckily, I was not carrying the extra 50 pounds around that long.
