One of the girls in my grad school program, “Bliss,” has lost a ton of weight this year. She told me that she’d been doing the San Francisco Weight Management program . Over the summer, my doctor recommended a program similar to this at Kaiser . This was the appointment when she told me I had slightly elevated blood pressure, and if I lost 20 lbs. Kaiser’s program uses the Optifast meal replacement shakes, bars, and soups. After you lose the weight, they offer a maintenance program and “weight coach” for up to 52 weeks. It would mean 4 months of eating the meal replacements, but if I started in March, I could lose a huge amount of weight by July. I know that true weight loss should be slow and steady, and I’m not looking for any quick fixes, but for the amount of weight I need to lose, I do feel that I need to do something to help me jumpstart it. She went on a program similar to this, lost a ton of weight really quickly, wore her skinny jeans, pulled out that wagon full of fat, and then gained it all back plus more. From Day 1 of the Optifast Diet to Success! (You don’t need to be a member of Kaiser to do the Optifast program they manage. They want to be sure the diet is appropriate for you – that you are obese, basically, and also that you don’t have other health problems that make the Optifast regime not medically advisable. I g0t 5 a day (which is the lowest amount “prescribed” and which totals 800 calories a day), so each week order 35 products. You are only supposed to have 1 or 2 soups a day (due to sodium), and can’t skip the shakes, since they have more vitamins I think. Some of the people I’ve heard can have as many as 10 products a day (so obviously they pay more for. Typically the folks with more food have two at a time, since you are supposed to eat every three hours, but not wake yourself up at night to eat your product! At the first group meeting (there are about 13 in our particular group) you get weighed (privately), your blood pressure is taken, and you put in your order for the first week of product – so you don’t actually start the diet for another week. You go for lab work the week before the med check. Nobody at our meeting shares how much they have lost, at least not to the whole group. I guess it’s bad form to share to the group in case others aren’t losing and you are? (In Weight Watchers they use to tell us each meeting after weigh-in how much total weight had been lost collectively by the group that week. She’s such a fan of the Kaiser Optifast program that she is now paying the cost for her adult son to also do the program, at a Kaiser in another town. One reason I think this is because I see that some of the search terms that people use when they land on my blog are for phrases such as “not losing weight on Optifast” and about plateaus on Optifast. I also know that some people do re-gain (as with all diets), and that another (big) obstacle is the cost. The opinion seemed to be that the non-food costs (for lab work, doctors and medical monitoring during Phase 1, program staff, etc.) are about $2700. Few Insurers Provide Coverage For Weight Loss Treatment

“Insurers will cover sleep apnea and diabetes, but not the treatment that could have prevented sleep apnea and diabetes,” two conditions linked to obesity, said Jennifer S. Lee and her colleagues reported in a recent study  that many states allow insurers to charge obese patients higher premiums or deny coverage of the condition altogether. They also found that only a handful mandate coverage for any of the three evidence-based treatment methods : bariatric surgery, medications, and nutritional assessment and behavioral counseling, which are classified as lifestyle measures. Researchers found that regular exercise and modest weight loss sharply reduced the risk of developing diabetes — more than medication did — a benefit that has persisted for more than a decade. The program covers up to four visits with a physician that focus only on weight loss and a maximum of six visits with a nutritionist annually, as well as the use of obesity drugs, which the insurance company’s chief medical officer, Don Bradley, said are little-used, in part because of concerns about their side effects. Physician Arthur Frank, founder and co-director of the GWU Weight Management Program, says the demand for proof that programs like his work is often used by insurers as a smokescreen to deny payment. Frank says the lack of insurance coverage is predicated on the view that nonsurgical treatment rarely works and that obesity is a matter of “personal misconduct,” not faulty neurochemistry.
